YOGA

I, the undersigned, understand I am participating in classes or services during which I will receive information and instruction about yoga and health. I recognize that yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness program, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities.

In consideration of being permitted to participate in instructional sessions in-person or through the internet, I agree to assume full responsibility for any risks, injuries or damages, known and unknown, which I might incur as a result of participating in these programs.

I understand Yoga Therapists and Yoga Instructors are not licensed under laws of this state to practice any form of medicine, they will neither diagnose nor prescribe for any condition or problem from which I may appear to be suffering, and that yoga is not a substitute for medical treatment. I understand that no guarantee or promises of cures have or will be made to me. I understand that Yoga Therapy and Yoga Instruction is an educational process for me to learn self-care techniques that I myself deem useful.

I understand that Dragonfly Yoga often records its instructors during yoga instruction, and I may incidentally be recorded when participating in classes. I hereby grant Dragonfly Yoga permission to use my likeness in a photograph, video, or other digital media used in promotional or educational purposes. I waive any right to royalties or other compensation arising or related to the use of this media.

MASSAGE

I understand that the massage service offered is for the therapeutic purpose of general wellness, stress reduction, and relief of muscular tension. I understand the risks associated with massage therapy include, but are not limited to superficial bruising, short-term muscle soreness, and exacerbation of undiscovered injury. If I experience any pain or discomfort, I will immediately inform my therapist so that the pressure or techniques can be adjusted to my comfort level. I will not hold my massage therapist responsible for any pain or discomfort I experience during or after the session.

I do not have any injuries or conditions that prevent me from receiving massage therapy. I understand the importance of informing my massage therapist of all medical conditions and medications that I am taking, and that there may be additional risks based on my physical condition.

I understand that I or the massage therapist may terminate the session at any time. I release the massage therapist and Dragonfly Yoga, LLC from all liability for any harm that may unintentionally result from this treatment. By signing this form I agree to the conditions as outlined above, and I release the massage therapist and business from all liability for any harm that may unintentionally result from this treatment.

I further understand that massage therapy is not a substitute for a medical examination or treatment, and that I should see a physician or other qualified health specialist for any mental or physical ailment of which I am aware. I understand that massage therapists do not diagnose illness or disease, and nothing said during the treatment should be construed as such. My consent is informed and voluntary and I understand that I may withdraw my consent at any time except for actions already taken.

COVID-19

Dragonfly Yoga, LLC cannot guarantee that you will not become infected with COVID-19. By attending in-person classes or services, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk you may be exposed to or infected by COVID-19 by attending classes and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand it is my responsibility to monitor my health and agree to not attend in-person classes should I feel sick or demonstrate any symptoms of illness. I have read, understand and will continue to monitor Dragonfly Yoga’s COVID-19 policy and protocols, which are available here.

DEADLINES AND REFUNDS

I understand and agree to the following cancellation deadlines and refund policies:

In-person and internet class and workshop credits, packages and purchases are NOT transferable, shareable, or refundable. There is a 6-hour cancellation policy for classes. Unlimited packages require a 3-month minimum subscription.

There is a 24-hour cancellation policy for private sessions, massage therapy and workshops. Any cancellation within 24 hours of the private session appointment time is subject to a cancellation fee equal to the normal cost of the appointment.

I am 18 years of age or older or have the signature below of my Legal Guardian. I hereby agree to irrevocably release and waive any claims that I have now or hereafter may have against Dragonfly Yoga, LLC or any of its Yoga Therapists, Yoga Instructors, Massage Therapists or staff.
